In-home Supports​


Our team of professional support workers are here to assist you in your homes to live as autonomously as possible. Offering companionship whilst assisting in general house duties such as: cooking meals, general cleaning/ sorting, grocery shopping and appointment attendance; we are here for you!!!!

Social Work & Other Therapies: Creative Arts, Animal Assisted Therapy &  ​Counselling

 

These services are available to individuals to better manage and build resilience when faced with grief and loss, adjustment to living with disability, anger, behavioural challenges, low self-esteem, heightened stress/ anxiety, depression, family & relationship conflict, identity issues, social isolation, and eating disorders. Sessions are promoted from a strength perspective with a solution based focus. Therapy may be inclusive of various techniques (talking, art/ expressive, animal & play) to achieve best outcomes and rapport with you as the participant.

Art and Creative Programs can contribute to fours key NDIS outcomes: choice and control, social engagement, economic participation and health & wellbeing. This technique allows you to have fun, build on personal interests, be creative, address anxiety or depression through expressive art techniques, whilst guided by a professional artist/ social worker

Animal Assisted Therapy assists you to feel calm when in a narratives therapy session with the social worker. Studies suggest that when focusing on patting a pet, it bring joys, calmness and most of all reduces anxiety when in a therapeutic setting. Animal therapy can also be useful in exposure therapy and addressing sensory issues. 

Counselling is available for family and participants to help adjust to situations they may struggle with in their everyday lives. Multiple techniques are used in these eclectic counselling sessions to best suit the needs of the client. Sessions are aimed at guiding the client to discover a solution to their problem, then together attend to the issue whist building resilience. 

Social Work is applicable to you if you wish to engage an advocate to help address your well-being and to identify external issues that you feel may create inequality, injustice and discrimination. Casework, counselling, and community engagement are all key when addressing issues at both your personal and social level. 

​Community Participation, Recreational & Skills Development

 

Learn new skills with individualized programs to build, developing or regain skills in a range of areas to ensure that you are as independent as possible. Aimed to create opportunities for people to develop and foster friendships and enable access to interesting community activities and events. 

We will help you get get organized through assistance to do your shopping, pay bills, attend appointments, study or start a business.

We can assist you in finding new hobbies or building on existing ones.

We are happy to link you to groups of your interest and attend with you for support if you desire (sporting, art, woodwork, self-care, animal, socials, quiz nights, gardening, cooking, exercising, gaming, etc )

 

There are many activities or events your community offers so why not get out there and try them?

Psychosocial Support

 

Psychosocial supports are non-therapeutic interventions that assist you to  participate in your community, manage daily tasks, undertake work or study, find housing, get involved in activities, and make connections with family and friends, if you have severe mental illness. Psychosocial supports are specific to the person and their needs, with the focus being to build capacity and networks when most needed. Psychosocial support is a short term support rather than ongoing, and typically you will have either support coordination OR psychosocial supports funded in your NDIS plan. Optimum Me can assist by helping you with the following:  

  • Psychosocial support interventions

  • Group based psychosocial interventions programs

  • Family and carer psychosocial supports 

During these sessions we can focus on: Developing social skills, friendships and relationships (with colleagues, family or friends), Building life skills, including confidence and resilience, Developing work goals, skills and qualifications, Assisting you to stay physically well (exercise, nutritional education, cooking, wellbeing practice), Finding and maintaining a home, Money management, and Support with drug, alcohol and smoking issues​

Get Aways & Holidays
Optimum Me Support Services can assist you in designing an individual holiday / get away around your needs and budget. Why go to traditional respite when you could be doing something you want to do?  Work with a member our team to design your  perfect holiday!
Although NDIS do not fund your holiday, you may have funding to for a worker to support you on your trip.
Short stays, camping, Interstate; it is all possible!
